How To Use This Form
- Step 1 | Tell Us About You
- Enter your first and last name.
- Enter your company name.
- Enter the email address where you can be contacted.
- Enter the phone number where you can be reached.
- Step 2 | About Your Cargo
- Enter a description of your cargo. This should be a general name that would refer to the whole lot ie. "Frozen Chicken", "Assorted Merchandise", "Steel Coil", "Paper Rolls", etc.
- How much of the cargo is remaining.
- Tell us the date the damage occurred.
- What was the value of the cargo before any damage occurred.
- What party rejected the cargo.
- What is the current location of the cargo.
- Is there a person that we may contact at the current location.
- Please describe the circumstances surrounding the loss.
- Have any sale restrictions been placed on the cargo.
- Are there any circumstances thay may delay the release of the cargo.
- Step 3 | Salvage Return
- Indicate how you would like us to proceed with salvaging the cargo.
- Enter a claim number or any reference that you would like to use when contacting you about this claim.
- Who will the proceeds be payable to when the sale is complete.
- The address that the salvage proceeds should be mailed to.
- Under some circumstances, ASL can pay charges on your behalf in order to expidite the salvage of the cargo. The charges paid by ASL will then be deducted from the return to you.
- Step 4 | Upload Documents
- Use this area to upload any photos, bills of lading, inventories, spreadsheets, etc.
- You may add multiple files at the same time. Simply click the button again after adding each file.
- File types accepted are: "jpg", "jpeg", "gif", "bmp", "pdf", "tif", "tiff", "xls", "csv", "doc", "rtf", "png"
